Dr. Jeremiah Church was raised in Utah and earned his Bachelors of Science at Southern Utah University. He later earned his Bachelors of Vision Science and his Doctor of Optometry from Nova Southeastern University College of Optometry, in Fort Lauderdale, FL. Upon graduation Dr. Church served his country for over seven years as an U. S. Naval Officer where he was Head of the Optometry Department, at U. S. Naval Hospital Oak Harbor, WA and later U. S. Naval Hospital Naples, Italy.

Dr. Church has been recognized for many accolades. To mention a few, he graduated Optometry with distinguished honors and was inducted into the Gold Key Honors International Society. Most recently, he received the very prestigious Navy and Marine Corps Commendation Medal (for the second time) for outstanding performance and achieving 99% overall patient satisfaction.

Dr. Church is an active member of both the American Optometric Association (AOA) and the Utah Optometric Association. He has also been a member of the Armed Forces Optometric Society, an executive member of Nova Optometric Practice Management Association, and member of the AOA Political Action Committee, which educates congressman on the role of optometry in the best practices of eye care.

Dr. Church also finds time to volunteer. He has volunteered his professional expertise by providing eye screenings to over 500 Special Olympic Athletes, as well as, provided eye screenings to over 150 underserved individuals with REACH Health Fair in Florida. He has also volunteered as a leader for the Boy Scouts of America and led Naval Air Station Whidbey Island’s efforts in one of the largest annual charity events in the Navy.

Dr. Church is happily married with three adorable children. They enjoy the great outdoors of all seasons and delight in spending time together taking road trips and traveling the world.
